Synopsis

The All-on-4® concept is an innovative surgical procedure for the rehabilitation of edentulous jaws focusing entirely on the patient. By taking advantage of the dense bone available in the anterior maxilla and between the mandibular foramina, this widely applicable surgical technique allows the treatment of a significantly greater number of patients, while avoiding the need for complex, time consuming and often debilitating bone grafting procedures.

An important aspect of the long-term success of the All-on-4® concept is the full-arch implant-supported dental prosthetics. There are several options for both the immediate provisional and the definitive prosthesis with the ensemble of different materials in the infrastructure, the artificial gingiva and the crowns, each option with its own advantages and disadvantages that warrant discussion. Some examples include the MALO CLINIC ceramic bridge, the MALO CLINIC acrylic bridge, the MALO CLINIC PEEK-acrylic bridge.

The presentations by MALO CLINIC will focus on the laboratory process, clinical outcomes and scientific research using different materials in full-arch implant supported prostheses (including zygomatic implants). During these two days, the MALO CLINIC clinical team pioneer of the All-on-4® concept will demonstrate how to achieve immediate function, excellent esthetics and predictable long-term success through state-of-the-art protocols including, surgery, prosthodontics, and maintenance. In this prosthodontic clinical residency, the participant will have the opportunity to watch live surgery, perform prosthodontic hands-on and take advantage of a prosthetic mentorship, watching live patient treatments/appointments and discussing the cases.

This course is targeted to oral surgeons, prosthodontists, or dental laboratory technicians that want to stay up to date with the latest protocols related to the All-on-4® concept.

This interactive course is structured to provide step-by-step training at a very personal level. It employs case-based learning by emphasizing evidence-based learning for daily usage. Participants will learn from the basic tools to the latest advancements in digital implant dentistry. Streamlined clinical and digital workflows will be presented to increase efficiency and clinical outcomes. The DTX Studio™ Clinic, DTX Studio™ Implant and DTX Studio™ Lab software will be presented, explaining how to simplify the digital workflow for predictable guided immediate implant placement and immediate loading. The digital implant planning rules for anterior, posterior and full arch cases will be reviewed and updated to achieve biological, functional and esthetic success. The attendees will improve their own confidence and skills with digital planning through immersive software hands-on.

Indications and limitations of the static and dynamic guided surgery will be reviewed and discussed, from simple flapless to advanced procedures, including digital assisted soft tissue sculpturing (DASS Technique), Guided Socket Shield protocols and X-Act Teeth. Special emphasis will be given to interdisciplinary digitally assisted treatment planning and final aesthetics in the different clinical scenarios.

Ever since Dr. Paulo Maló published the first article on the All-on-4™ protocol for implant reconstruction of the mandibular full arch in 2003, there have been significant changes in the way dental professionals managing patients for full-arch implant restoration. Initially, there were concerns regarding the use of only four implants to support a full arch fixed restoration. An ideal treatment plan requires thorough esthetic, functional, and structural analysis, supported by radiographic and digital examinations. Necessary adjustments to optimize teeth and soft tissues are transferred to the technician to create a wax-up and a diagnostic/functional mock-up. With 3D technology, it is now possible to create a patient “avatar,” enhancing diagnosis, planning, communication, and interdisciplinary workflows — applicable to both esthetic cases and complex full-arch rehabilitations. In implant therapy, diagnosis and treatment planning is still more demanding due to the critical aspect of the soft tissue contour and volume.

The success of implant-supported prostheses depends on optimal integration of all interfaces prosthetic, esthetic, soft tissue, and bone — through well-chosen surgical and restorative protocols. A solid grasp of biological principles and proper material selection are essential for achieving a stable mucosal seal and natural esthetic outcomes. Perfect esthetic integration relies on three pillars: diagnosis and prosthetic planning, proper management of the bone compartment, and careful handling of the supracrestal soft tissue compartment. The latter, increasingly important in modern implantology, is critical for avoiding complications and major esthetic defects.

Safe treatment in esthetic zones begins with thorough evaluation and selection of the most suitable surgical-prosthetic strategy. Finalization requires in-depth knowledge of restorative materials to replicate in the definitive prosthesis all the features tested in provisionals. Close collaboration with the lab and the use of digital workflows improve quality and efficiency. Cementation — especially adhesive protocols — is a critical phase, with specific guidelines for glass ceramics and zirconia. New ceramic materials and adhesive techniques have reshaped restorative dentistry, promoting minimally invasive preparations that preserve enamel and tooth structure.
