The All-on-4® treatment concept: A two-decade evolution

When Dr. Paulo Maló took the stage at Nobel Biocare EAO 2013 — the 15th anniversary of the first placement using the All-on-4® treatment concept — the doctor repeated a question he had heard from other clinicians: “Why did it take so long?” At this point in time, the All-on-4® treatment concept was just starting to become commonplace in the United States. Dr. Maló gave context to the innovation of the All-on-4® treatment concept, especially subperiosteal implants, which often required anywhere from 6 to 18 implants to support a full-arch rehabilitation. Digital innovations and guided surgical protocols continue to make the All-on-4® treatment concept accessible to more clinicians and patients.

Introductory concepts for the zygomatic dental implants — Dr. Shuaib Malik

Traditionally, a severely resorbed maxilla with sinus pneumatization would need multiple surgeries including sinus augmentation/lifts and bone grafts to rebuild enough bone mass to create a sufficient foundation for traditional dental implant placement. In severely resorbed cases, zygomatic implants have been an evidence-based surgical and prosthetic solution since 1999 and have eliminated the need for bone grafting altogether.

Edentulous patients and patients with failing dentition who want a fixed prosthetic rehabilitation are very often faced with time-consuming, expensive, and uncomfortable bone grafting procedures. The perceived complexity of rehabilitation is a great obstacle to treatment acceptance for these patients and many dental clinicians as well. The All-on-4® treatment concept was developed by Nobel Biocare to eliminate many of the difficulties facing patients and clinicians. It allows for an immediate-function, full-arch, implant-supported prosthesis without the need for any bone grafting procedures, even in atrophic situations.

In a short time the attendees will observe complete rehabilitation of the patient, by delivery of a highly functional and esthetic solution supported by four implants in anatomically strategic positions and angles, thus dramatically increasing the quality of life for the patient.
