Nobel Biocare supplies all implantable devices with an Implant Card/Patient Medical Device Card which contains important information for patients regarding the device.​

To request a hard copy, please contact your local sales representative.​

​Complete the Implant Card/Patient Medical Device Card by filling it out with the patient- and device- specific information as indicated and provide the completed card to the patient.
