Professor Paulo Malo has been successfully treating patients with the All-on-4® treatment concept for decades. Here, he gives his guidance to those looking to incorporate this proven approach in their practice.
For anyone that is starting with the All-on-4® treatment concept, I have four main pieces of advice:
Number one is to sign up for proper training. Anyone thinking about starting with the All-on-4® treatment concept should first attend a comprehensive course. This should be the initial step on what can be a very rewarding journey to changing patients’ lives with this proven protocol.
My second recommendation, once you are introducing the All-on-4® treatment concept into your practice, is to make sure that you use the right implant. You need an implant system that is backed by many years of clinical evidence. You should also be sure to use restorative components that are designed to fit that implant. This is very important when you consider that the All-on-4® treatment concept is only four implants supporting a prosthesis with cantilevers. This means there is significant stress on that structure. Using the right components helps prevent problems occurring at a later date.
Third, make sure that you use a CAD/CAM restoration from a high-quality provider like NobelProcera. Again, the stresses on the prosthesis need to be managed, so passive fit is crucial for long-term success.
The fourth and final tip is to make sure that you incline the posterior implants as much as possible, up to a 45-degree angulation. This is because the more you incline the implant, the more you reduce the cantilever. This can make a big difference to the biomechanics of the structure, and can help ensure a successful result for your patients.
