A custom-made product is a product fabricated especially for a named patient according to specific characteristics and prescribed by an appropriately qualified doctor, who assumes full responsibility. Clinicians must order and specify each Custom-made Device via a prescription form. Each request is only valid for one patient and for one indication or treatment. Repeat production of identical products is not possible for regulatory reasons. Because of the customized nature of the service, Nobel Biocare is unable to provide catalogs or product lists.
How to order a Custom-made Device
To order a Custom-made Device, the treating practitioner should first contact the customer support representative at their local Nobel Biocare subsidiary to discuss the issue. If the conclusion reached is that a Custom-made Device is required, the treating practitioner should complete the online prescription form and follow the instructions below.
The order form has all fields mandatory except fax.
When the order form has been completed and signed, the practitioner must send the order either by fax or e-mail (scanned copy) to their local Nobel Biocare customer support representative who will then forward it to Custom-made Device Management in Gothenburg. Custom-made Device Management will then review the requirement and assess whether it fulfills the criteria for Custom-made Devices.
Practitioners will be advised of the outcome in due course.
Generally this service is available to customers in EU, US, Canada, Australia, Liechtenstein, Switzerland and Norway. If in exceptional cases service for a customer in another country is envisaged the customer needs to contact the local NB office for a legislative feasibility check.
